Overview
- Goal 2: Improve the prevention, diagnosis, treatment, and management of COPD by improving the quality of care delivered across the health care continuum.
- Goal 4: Increase and sustain research to better understand the prevention, pathogenesis, diagnosis, treatment, and management of COPD.
- Increase public awareness of the risk factors and symptoms of COPD so that earlier diagnosis of symptomatic individuals becomes the norm
- Develop a unified, multidisciplinary educational curriculum for health care professionals, including primary health care providers, using harmonized clinical practice guidelines
- Help expand, coordinate, and optimize COPD research efforts
- Promote research that can lead to strategies that help prevent the onset and progression of COPD and improve the quality of COPD care and management
Activity Description
The Emphysema Foundation of America's STRIVE program is a peer support group for individuals with diagnosed Chronic Obstructive Pulmonary Disease (COPD). Participants are provided with tools and resources to improve disease self-management. This need is especially great among underserved populations who face multiple challenges when trying to understand and manage their chronic lung disease. This program is based on the theory of planned behavior that links beliefs to behavior control and intention.Program's Goal:
Participants to gain greater confidence in COPD self-management.
Participants to experience improved daily living and greater quality of life.
Participants to report maintaining a regular habit of using a COPD action plan and symptom checker and updated medication list.
Participants will have improved communication with the provider and healthcare team.
