Activity Description

The toolkit offers evidence-based and promising models and resources to help rural communities, healthcare providers, and other organizations develop and implement chronic obstructive pulmonary disease (COPD) programs in rural communities. This toolkit aligns with Goal 1 - Objective 2 of the COPD National Action Plan by sharing information and resources to increase the effectiveness and variety of activities using evidence-based approaches to raise awareness of COPD and reduce its burden. The toolkit also relates to Goal 2 - Objective 5 to improve access to care for people with COPD, particularly for those in hard-to reach areas.
