Activity Description

The Emphysema Foundation of America's STRIVE program is a peer support group for individuals with diagnosed Chronic Obstructive Pulmonary Disease (COPD). Participants are provided with tools and resources to improve disease self-management. This need is especially great among underserved populations who face multiple challenges when trying to understand and manage their chronic lung disease. This program is based on the theory of planned behavior that links beliefs to behavior control and intention.

Program's Goal:
Participants to gain greater confidence in COPD self-management.
Participants to experience improved daily living and greater quality of life.
Participants to report maintaining a regular habit of using a COPD action plan and symptom checker and updated medication list.
